Use of sulfonylureas and insulin was associated with an increased risk

To determine the association between antidiabetic drug use, diabetes
and the risk for pancreatic cancer, researchers conducted a case-
control study of drug prescription, diagnostic, hospitalization and
fatality information collected by the British General Practice
Research Database (GPRD) between 1995 and 2009.

The study included 2,763 patients aged younger than 90 years diagnosed
with pancreatic cancer vs. 16,600 participants without pancreatic
cancer. Short-term use of metformin or sulfonylureas and/or insulin
exhibited no significant impact on pancreatic cancer risk, according
to study results. However, long-term use of antidiabetic medications
demonstrated notable effects on pancreatic cancer risk in the diabetic
demographic.
The use of sulfonylureas (>30 prescriptions; adjusted OR=1.9; 95% CI,
1.32-2.74) and insulin (>40 prescriptions; adjusted OR=2.29; 95% CI,
1.34-3.92) were connected with a heightened risk for pancreatic
cancer. Although long-term use (>30 prescriptions) of metformin was
not associated with a significant risk for pancreatic cancer (OR=0.87;
95% CI, 0.59-1.29), long-term use of metformin was linked to a
decreased risk in women (OR=0.43, 95% CI, 0.23-0.8).

Alzheimer’s drug developer Satori Pharmaceuticals raised $15 million
last week, bringing its total venture capital raised to $37 million—an
impressive haul, considering the Cambridge, MA-based company doesn’t
expect to begin human testing of its lead compound until early 2013.
But what may be even more notable is that Satori’s investors are
betting on an entirely new source of Alzheimer’s-fighting compounds:
black cohosh, a flowering herbaceous plant that’s most commonly
marketed as a nutritional supplement to relieve symptoms of menopause,
such as hot flashes.
Satori was founded in 2005 around a discovery made at the Mayo Clinic
in Jacksonville, Fla. Scientsts there separated out and then
scrutinized the components of black cohosh, until they hit on one that
would act against amyloid beta 42—a protein believed to cause the
damaging brain plaques that characterize Alzheimer’s. The key
attribute of the compound the Mayo researchers found, says Satori CEO
Jeff Ives, is that it appears to inhibit the toxic protein without
interrupting normal brain functions. “They just went hunting for all
the compounds no one has ever characterized—the novel structures,”
Ives says. “The extract they came up with looks like dark purple
stuff, but it’s made in heaven.”
Amyloid has long been a sought-after target in Alzheimer’s drug
development, but it’s been a challenging one to tackle. Most attempts
to reduce the buildup of brain plaques have sought to inhibit a family
of amyloid-promoting enzymes called secretases. Problem is, secretases
are present in cells all over the body and are necessary for various
functions, so broadly blocking them can cause dangerous side effects.
In 2010, Eli Lilly stopped developing a secretase inhibitor it was
working on because it seemed to increase the risk of skin cancer in
patients participating in its trials. “You can’t just take a hammer to
these enzymes,” says Jeff Jonker, Satori’s chief business officer.
“There are places in the body where they are needed for normal
functioning.”
In animal studies, Satori’s compound has proven to be highly selective—
meaning it binds with a specific enzyme called gamma secretase, in
turn blocking amyloid beta 42 without affecting normal proteins. The
company hopes to be able to intervene early, maybe even before
patients have symptoms, to kill the “bad actor” in Alzheimer’s, Ives
says. “What we want to do is stop the first step in the formation of
plaque,” he says.
Black cohosh is a perennially blooming plant that has been used since
the 1800s to treat everything from sore throats to depression. Natural-
medicine practitioners believe it’s the estrogen-like compounds in the
plant that relieve menopausal symptoms. But the plant’s Alzheimer’s-
fighting properties are not related to estrogen, Ives says. “The
molecule found at Mayo is a minor constituent of black cohosh that
modulates gamma secretase,” he says. “It’s a unique chemical series.
Satori’s staff of 11 scientists took the raw material from Mayo and
spent years figuring out how to make it into a safe, effective drug,
Ives says. They used medicinal chemistry techniques to produce a
molecule that could get beyond the notoriously impenetrable blood-
brain barrier, and that could be made into a drug patients can take by
mouth. Satori plans to file an application to the FDA later this year
for permission to begin human trials.
Much of Satori’s development strategy hinges on a multifaceted plan to
prove out the molecule’s attributes in animal models, says Chris
Ehrlich, general partner at InterWest Partners in Menlo Park, CA, one
of the startup’s original investors. That’s because Satori wants to
attract a deep-pocketed
development partner as early in the process as possible. “Our
hypothesis is if we can prove out pre-clinically that the molecule
behaves, in multiple animal species, there will be partners who are
interested before the drug ever sees a human being,” he says.
Satori’s management team is so intent on signing on a Big Pharma
development partner early that they hired Jonker two years ago and
sent him out to cultivate relationships that may lead to development
deals. The company is very open, Ehrlich says, about how it’s running
the animal studies, and it regularly seeks feedback from potential
pharma partners. “We say, ‘Here’s what we’re doing, tell us what you
need to see. We’ll build it if you come,’” Ehrlich says.
A number of pharmaceutical and biotech companies are trying to develop
more effective treatments for Alzheimer’s, which is considered among
the world’s biggest unsolved medical problems. More than 5 million
people in the U.S. alone suffer from the disease—a figure that’s
expected to skyrocket with the aging of the population. One of the
most widely watched experimental treatments is bapineuzumab, a drug
being developed by Pfizer and Johnson & Johnson that’s now in late-
stage trials. And last December, Merck unveiled an early-stage
compound that works against an amyloid-promoting enzyme called BACE.
Ives believes that even if those companies don’t want to partner with
Satori, the more competitors in the Alzheimer’s space the better. “I
expect if Merck is successful with a BACE inhibitor we could have a
combination product,” he says. “It’s two different ways of skinning
the same cat—we’re both shutting off these neuro-toxic peptides.”
But Satori has a long way to go to prove its compound works. Because
its drug will be designed to be taken chronically, starting at the
early stages of the disease, Satori will have to run large and lengthy
trials to prove it’s safe. “This is a tough population,” Ives says.
“They’re in their seventh or eight decade of life and on a multitude
of drugs for treating other things like heart disease and arthritis.
We don’t want drug interactions. This has to be mother’s-milk clean
and safe to be taken for years.”
InterWest Partners, New Enterprise Associates, and Prospect Venture
Partners led Satori’s most recent funding round, which will be
deployed towards the company’s goal of collecting enough pre-clinical
data to generate a Big Pharma deal. Jonker and Ives say they expect to
have some data ready to present at the industry’s big annual confab—
the Alzheimer’s Association International Conference in July. Although
they have a bit more work to do, they remain confident someone will be
willing to take on the risk of developing their black-cohosh derived
drug. “There’s been a lot of interest,” Jonker says. “The magnitude of
this disease in terms of its impact socially and economically,
relative to the drugs the industry has been able to produce is out of
whack. This is the largest unmet medical need right now.”

In the 19th century, black cohosh was a popular treatment for a range
of ailments, including sore throats, depression and menopause. But
Xconomy explains how investigators at the Mayo Clinic found a molecule
in black cohosh that can modify gamma secretase. And the biotech
startup Satori Pharmaceuticals--which raised $15 million last week--
thinks it can ultimately prove to be a safe and effective treatment
for Alzheimer's disease.
As Xconomy's Arlene Weintraub explains, one of the big issues with
experimental drugs that target secretases as a way to prevent the
buildup of amyloid plaque in the brain is that secretases play a big
role in human health. Eli Lilly ($LLY) found out the hard way that a
sledgehammer approach could trigger disease, which was the kiss of
death for that particular program.
Satori, however, takes a much more selective approach. Its molecule
binds with gamma secretase, singling out the amyloid beta 42 for a
full-body tackle and leaving the rest of the proteins free to promote
good health. And that, executives say, is the key to preventing the
buildup in plaque, stopping the disease before it takes hold of its
victim. That thesis, says Chris Ehrlich, general partner at InterWest
Partners, which invested in Satori early on, should quickly win over a
pharma partner.

Currently, there's no consensus on what causes Alzheimer's or how it

it has a drug that works.

Abstract
Cimicifuga racemosa (black cohosh) is commonly used in traditional
medicines as treatment for menopausal symptoms and as an
antiinflammatory remedy. To clarify the mechanism of action and active
principle for the antiinflammatory action, the effects of aqueous C.
racemosa root extracts (CRE) and its major constituents on the release
of the proinflammatory cytokines IL-6, TNF-alpha, IFN-gamma, and the
chemokine IL-8 were investigated in lipopolysaccharide (LPS)-
stimulated whole blood of healthy volunteers. CRE (3 microg/microL and
6 microg/microL) reduced LPS-induced release of IL-6 and TNF-alpha in
a concentration- and time-dependent manner and almost completely
blocked release of IFN-gamma into the plasma supernatant. Except for
IFN-gamma, these effects were attenuated at longer incubation periods.
IL-8 secretion was stimulated by CRE. As shown by quantitative real-
time RT-PCR, effects on cytokines were based on preceding changes in
mRNA levels except for IL-8. According to their content in CRE, the
phenolcarboxylic compounds caffeic acid, ferulic acid, and isoferulic
acid, as well as the triterpene glycosides 23-epi-26-deoxyactein and
cimigenol-3-O-xyloside, were tested at representative concentrations.
Among these, isoferulic acid was the prominent active principle in
CRE, responsible for the observed inhibition of IL-6, TNF-alpha, and
IFN-gamma, but not for IL-8 stimulation. The effect of this compound
may explain the antiinflammatory activities of CRE and its beneficial
actions in rheumatism and other inflammatory diseases.

Cimicifuga species have been used as traditional medicinal herbs to
treat inflammation and symptoms associated with menopause in Asia,
Europe, and North America. However, the underlying mechanism of their
anti-inflammatory effects remains to be investigated. With bioactivity
guided purification involving the use of partitioning extraction and
high performance liquid chromatography, we isolated one of the key
bioactive constituents from the rhizome extracts of Cimicifuga
racemosa. By NMR spectroscopy, the molecule was identified to be
cimiracemate A (1). This compound (140 muM) suppressed the
lipopolysaccharide-induced TNF-alpha production in the blood
macrophages by 47 +/- 19% and 58 +/- 30% at LPS concentrations of 1 ng/
mL and 10 ng/mL, respectively. The anti-inflammatory activity of
compound 1 may be due to its modulation of a signaling mitogen
activated protein kinase and transcription factor nuclear factor-
kappaB activities. Compound 1 was found in other Cimicifuga species.
Our data indicate that compound 1 or its chemical analogues may have
the potential to be further developed as a new class of therapeutic
agent.

When cancer expert Dr. Nicholas Gonzales spoke of Steve Jobs, he
questioned why Jobs would receive a liver transplant considering his
cancer was not a primary liver cancer and must have spread from the
pancreas to the liver.
Once a transplant is performed, patients must take immune suppressants
for the balance of their lives. “That’s not good when you have a
history of cancer, because immunosuppression can stimulate cancer
growth because you’re suppressing your own immune ability to fight
cancer,” Gonzalez said.
If the medications Jobs was prescribed caused liver failure, the
painful ordeal of a transplant may have given Jobs a few more months
to live, the doctor added.
Dr. Joseph Mercola interviewed Gonzales in hopes of helping others who
are struggling with similar situations. There is no question Gonzales
believes he could have helped Steve Jobs.
Gonzales is not a famous cancer specialist, but he’s the very best
when you ask his patients, most of whom are living longer than
expected. He has a three-pronged approach to cancer treatment with
individualized programs for each patient. His first approach is to
discover the metabolic function of the patient. He then creates a
specific diet for that patient. He uses pancreatic enzymes, vitamins
and minerals, as well as his other protocols.
“Again, we’re precisely prescribing for each patient, and doses vary
from patient to patient,” Gonzalez said. “We don’t think that vitamins
and minerals have a particular anti-cancer effect, but we once treated
cancer specifically with large doses of pancreatic enzymes, which
conventional medicine thought only helped with digestion. There’s
evidence that goes back a hundred years that those pancreatic protein
digestive enzymes — the same enzymes we use every day to digest our
protein foods — have an anti-cancer effect and may represent the
body’s main defense against cancer. My average cancer patient probably
takes 10 capsules of pancreatic enzymes a day.”
We are learning that proper digestion can be the key to preventing
many diseases and assuring the body is able to utilize nutrients as
well as fight back against disease.
Gonzales’ third component is a careful detoxification plan. He
believes the body must be rid of toxins from dying tumors.
“You kill a tumor, that’s wonderful,” he said. “You get all the tumor
debris floating around the body. It’s quite poisonous.” He believes
the body will heal more efficiently when toxins are carefully
eliminated.
Gonzales spoke of one of his patient’s with the same type of
pancreatic cancer as Steve Jobs. Mayo Clinic advised the patient he
was terminal.
“He started with me in 1995, shortly after his diagnosis,” Gonzales
states. “He’s alive and well now, 16 years later. CT scans beginning
around 2000, showed total resolution of his large tumors. He had a
huge tumor in the pancreas.”
Gonzales is exceptionally unique in that he once treated patients
conventionally. He explains that he changed his entire lifestyle and
began to eat only healthy organic foods and supplements and couldn’t
believe the difference it made in his own life. He then decided to get
his patients to do the same. He admits that not every patient is
healed, but many of his have total success and those who do not live
longer than most patients with pancreatic cancer.
Gonzales talks about how movie stars are highlighted as they go to
“famous” physicians for cancer treatment that is known to be
unsuccessful, but used regardless. Methods that result in the deaths
of many of the stars are used, but not with Suzanne Summers. She used
alternative methods.

Abstract
BackgroundForkhead Box P1 (FOXP1) has been described as both a tumour
suppressor candidate and a potential oncogene. The aim of this study
is to identify new prognostic biomarkers and therapeutic target
structures for the diagnosis and treatment of hepatocellular carcinoma
(HCC).MethodsThe expression of FOXP1 mRNA in HCC was characterised
using real-time PCR and 20 pairs of fresh frozen HCC tissues and
corresponding non-cancerous tissues. FOXP1 protein expression in HCC
was confirmed using immunohistochemistry on a tissue microarray chip.
Finally, FOXP1 expression was correlated with conventional
clinicopathological features of HCC and patient outcome.ResultsThe
expression of FOXP1 mRNA and protein in HCC cells was much higher than
in normal hepatic cells (Z=2.315, p=0.021 and χ(2)=28.071, 95% CI
0.233 to 0.480, p<0.001, individually). The comparison of
clinicopathological characteristics and immunohistochemistry by χ(2)
test analysis showed that the high expression of FOXP1 in HCC was
related to large tumour diameter (χ(2)=6.210, p=0.013), high serum α-
fetoprotein levels (χ(2)=6.920, p=0.031) and later stage grouping with
tumour node metastasis classification (χ(2)=6.714, p=0.035). Kaplan-
Meier survival and Cox regression analysis showed that high FOXP1
expression (HR=2.182, 95% CI 1.146 to 4.154, p=0.018) and regional
lymph node metastasis (HR=2.326, 95% CI 1.037 to 5.217, p=0.041) were
independent prognosis factors.ConclusionsFrom this investigation the
authors elucidated for the first time that the correlation of high
FOXP1 expression correlates with an aggressive malignant phenotype and
may constitute a novel prognostic factor for HCC. These results also
support a role for FOXP1 as an oncogene in HCC.

Abstract
Dendritic cells (DCs) are responsible for priming T cells and for
promoting their differentiation from naive T cells into appropriate
effector cells. Emerging evidence suggests that neurotransmitters can
modulate T cell-mediated immunity. However, the involvement of
specific neurotransmitters or receptors remains poorly understood. In
this study, we analyzed the role of dopamine in the regulation of DC
function. We found that DCs express dopamine receptors as well as the
machinery necessary to synthesize, store, and degrade dopamine.
Notably, the expression of D5R decreased upon LPS-induced DC
maturation. Deficiency of D5R on the surface of DCs impaired LPS-
induced IL-23 and IL-12 production and consequently attenuated the
activation and proliferation of Ag-specific CD4(+) T cells. To
determine the relevance of D5R expressed on DCs in vivo, we studied
the role of this receptor in the modulation of a CD4(+) T cell-driven
autoimmunity model. Importantly, D5R-deficient DCs prophylactically
transferred into wild-type recipients were able to reduce the severity
of experimental autoimmune encephalomyelitis. Furthermore, mice
transferred with D5R-deficient DCs displayed a significant reduction
in the percentage of Th17 cells infiltrating the CNS without
differences in the percentage of Th1 cells compared with animals
transferred with wild-type DCs. Our findings demonstrate that by
contributing to CD4(+) T cell activation and differentiation to Th17
phenotype, D5R expressed on DCs is able to modulate the development of
an autoimmune response in vivo.

noted for years -- patients with positive things in their lives and a
positive outlook often respond better to treatment.
Although the signaling substance performs other functions, it's best
known as the "reward chemical" that surges in the brain whenever we
experience something pleasurable, from eating a favorite food to
kissing a sweetheart. The drug also plays a role in addiction.
But a team led by Dr. Sujit Basu of OSU's James Cancer Hospital has
found that dopamine has some powerful effects on blood vessels in and
around tumors.
Since he started studying dopamine at the National Cancer Institute of
India a quarter-century ago, Basu has learned that some receptors for
the chemical slow tumor growth by suppressing blood-vessel growth,
while other brain transmitters like epinephrine improve blood-vessel
and tumor growth. He first reported signs that dopamine could
influence the impact of drugs on breast- and colon-cancer cells in the
lab.
In their latest findings, the researchers demonstrated that injecting
dopamine into tumors greatly improves the size and structure of blood
vessels inside the tumors. Normally, blood vessels inside the abnormal
masses are jumbled, unstable and leaky, making it much harder to
deliver cancer-fighting drugs where they're most effective.
Basu and his colleagues injected dopamine into lab animals with colon
and prostate tumors and found surprising results.
"It worked like a wonder," Basu said in an interview. "It not only
normalized the blood vessels, it increased the blood flow. We were
able to double the amount of anti-cancer drugs inside the tumor."
The scientists point to the experience of Sandra Stockhorst, a colon-
cancer patient who saw tumors reappear despite surgery and one round
of chemotherapy early in her four-year battle with the disease. Her
doctors faced a challenge in getting drugs into the hard-to-reach
tumors.
Stockhorst said in a videotaped interview provided by OSU that she
believes her love of her horses helped her beat back cancer. "They
were a huge motivation," she said. "I would think 'I'm going to walk
to the barn today. I'm going to groom a horse today.' And I found the
strength to do it."
Not everyone produces dopamine at the same pace or even responds the
same way to pleasurable events. But Basu said it would be simple and
cheap to supplement the chemical for cancer patients.
"A dose costs 33 cents. When you compare it to most other drugs being
used in cancer therapy, it's really, really inexpensive,'' he said.
"And we know it's safe. Dopamine has been used in the clinic for many
years now -- in the brain and more recently to treat a number of
gynecological conditions. The side effects are well-known and
manageable."
So while human tests of the effectiveness of cancer treatment with
dopamine may be required, little or no new safety testing should be
needed. The researchers expect trials to prove effectiveness against
tumors in humans to start soon. In the animal tests, Basu said
dopamine allowed the cancer drugs to kill tumor cells without doing
any damage to nearby healthy cells.
"We've shown this can work in principle,'' Basu said. "Now we just
need to get this into the clinics to treat cancer patients."
