Two of my neighbors are long time diabetics.

One has been told to check her BG only in the morning, before eating.

The other was told to check several times a day to see what effect food
and exercise have on her system.

Seems like such diverse opinions cannot serve both well.

Is such disagreement common?

The question was not about what they were doing, but what they were
advised to do.

Okay. But what they are advised to do and what they think they are advised
to do can be to different things.

And frankly I thought it was a very stupid question. Of course they are
given different advice! Not all Drs. believe the same things. I had a
nurse advise me to eat lots of soup and fruit. Isn't that rather silly
advice? I think so.

There is no one type of diabetes. And even if we say there are 2 or 3 or 4
different types, there are all kinds of variations on that. A person who
uses insulin would most likely be told to test more frequently than one who
does not. Just as a person who has a lot of hypos would.

At some point there is really no point to test several times a day. I know that eating a cheeseburger and fries is going to run up my BG. I know that I can eat Barilla linguini in moderation and my BG will be fine. Why waste a test strip? I normally test only in the morning. Only if I get an unexpected reading will I test further during the day.

Every Dr will give different advice, that doesn't make them right. The VA
Dr tells me only to test every other day, and will only give 30 strips every
2 months. My previous Dr told me to test 3 times a day, but would only write
a script for 30 strips a month, my 89 year old mother, who is on Insulin,
was told by her Dr she only needed to test once a day. The medical
profession does not give the same advice to everyone. If your car mechanic
gave the type of advice your Dr does, he would be out of business in a very
short time.
You need to step back, and say, What is right for me. If the Dr won't write
the script, you have 2 choices, another Dr, or buy your own.

Testing only for fbg is good for the doctor, but if you want to gain

rigorous testing is required, at least at the beginning.

Whether peope follow the doctor's advice is another question, but many
doctors do not seem to think the rigorous testing , particularly in early
days, is owrth it or that the patient will pay any attention and prefer to
recommend a boiler-plate eatign pattern.

I asked the gal who only tests once daily, why she didn't test more.
Her reply was : Because it only upsets me to see those high numbers
after eating.
Now that one stumps me all together.
So obviously, her being told that there was no reason for multiple tests
suited her perfectly.

Apparently, doctors just have different views on the best way for their
patients to manage the condition.

Different people have different levels of diabetes. It is very likely
that correct instructions have been given to both folks. For example, a
well controlled Type 2 may only need to check in the mornings whereas a
Type 1 or a Type 2 whose diabetes has progressed to the need for insulin
would need to check many times per day.

All diabetics are not the same. Some need to check several times
a day, and for others once a day is enough.

I began at 5-10 times a day. Now that I am under control, I only
check in the morning.

It depends how concerned you are at what your bg is doing in what
circumstances. I check between once a day and several times a day, with an
emphasis on when I try a new food, a new exercise routine, or if the
previous reading was unusually high or low.

As a result I can now estimate what bg I will get when with a reasonable
level of accuracy, and I can determine what to eat and how to exercise to
control my bg.

It is vital to measure bg several times a day when on Insulin.

It is the caloric content, not the avoirdupois content, of one's daily
food, which determines whether one is overeating or not.
And not all foods have the same caloric content per oz.

So that Chung is wrong even about basics in his own speciality.

Thus following Chung's advice could be as dangerous to one's health as
believing him is dangerous to one's sanity.

How does she know what anyone's A1c is?

None of my pharmacists know any of my lab results, & unless I've mentioned
it to any in particular they don't know I've had cancer either.

Of the many diabetics I have spoke to, I have met only a few who could tell
me what the term "HbA1c" meant, let alone what the figure was. Either that,
or they were too embarrassed to say.

So maybe that doctor, rather than being guilty of malpractice, merely
understood the patient's personality and the futility of dispensing
advice that would be ignored.

When I went to the V.A., what the doctor said was irrelevant.

My V.A. doctor, in spite of my protest, said my A1c (8.2) was excellent
and I shouldn't abuse my fingers by testing more than once a week.

The pharmacist looked at the script, said V.A. policy is every other
day, and dispensed according to policy.

Sorry to say, the lab report agreed with the doctor--except for
misspelling "EXCELLANT"

So if her memory is accurate, none of the ones who told here were under
that. Some of the ones under that may not be coming to the chemist and
others may not have volunteered the information.

BUT, I suspect the statement is not for from accurate.

In my very worrying weeks after my initial diagnosis, the best information I
received was, believe it or not, from a security guard. He had been a
hospital nurse in his previous job and made a point of learning about
diabetes.

I am prescribed 6 test strips per day, much to the dismay of pharmacists
and Medicare. As part of the justification for the
prescription, my physician submits data that justifies the number
of strips he prescribes (at my urging.) I have no doubt that my
pharmacy knows my A1c and all other relevant facts. I also had
to sign an affidavit that I am the person using all the test
strips issued under that prescription. In order to get the
numbers to work out accurately, they issue me 100 days worth
of strips at a time.

Over the years? Certainly.
Also lots of doctors (whom I do keep informed).

I usually get my meds at the same chain, but not always at the same
branch, depending on what it is, where I am, how soon/fast I need it, how
much extra trouble it is to go to my "usual" place, etc.

Not only have I changed pharmacies at one time or another (because of
closure, preference, convenience, etc.) there are multiple pharmacists at
any one pharmacy (I am not talking about clerks, but actual pharmacists) so
even in the same store I may see different people from one time to another.
Then there are also various mail order arrangements I may have from time to
time.

Of course, I can't really *know* that they haven't gotten the information
somehow, but none has ever mentioned it to me, commented about maybe a
different med would be "better" or in any way indicated they had such
personal knowledge.

One did comment in astonishment once (about 20 years ago) at how much a med
cost -- the monthly price for critical med for my *dog* -- he couldn't
believe I'd spend $50/month! *For a dog!* I just said it was either that or
a one-time $25 shot of the pink stuff, which pretty much shut him up about
that!

Sort of off the topic, but as far as having multiple pharmacists go, some of
the uninsured shop around for the best value and then transfer, resulting in
multiple pharmacies too. Recently, Wal-Mart was ripping a friend off for
generic Vicodin by charging 35.88 for 60 pills. Walgreen's charges 29.99,
Rite-Aid wouldn't say unless she transferred, but Safeway charges 24.49
which is quite a difference. She transferred and got a 25.00 off certificate
for the next time she spends 50.00 on groceries. We were very surprised that
Wal-Mart was the most expensive. It pays to shop around if you don't have
insurance.

Even with insurance, here they offer things like a $25 or $30 gift card for
transferring prescriptions. I got the $30 one when I switched to Bartells.
Rite Aid offered me $30 to switch back but I didn't because I had some
problems with them. We have at times used Walgreens only because the have
one location that is open for 24 hours. They stopped taking my insurance
for a while but I think they are taking it again now.

Even with insurance, often Walmart is less expensive than the copay on
insurance covered prescriptions at my local pharmacy. YMMV

Its different now (cutbacks), but they used to warn me if sth didnt
make sense qua medication. Not anymore now.

We are making cutbacks right now ourselves, the first thing we see is
that our 2 cats are the most costly persons in our household :-)
Not that we will alter their food and vet's visits!

As of late lots of animals are dumped in shelters, because people cant
pay for the illness of their pets :/

I find htat for several of my generics I do better with CVS 3 month
special price and it doesn't puch me towards the donut hole, which may be
reduced, but is not gone at this time.

How do you think they got to be #1?
I usually stay away from market leaders, so I hardly ever shop at
Walmart. I *did* notice though they advertise so-called "price rollback"
and in many cases those prices are quite a bit higher than those of
other retailers.
I used to shop a Sams Club membership through my company and used to
compare their merchandise with Costco. Rule of thumb was: whenever they
(Sams) carried the same item in the same quantity, they'd be 5c or 10c
cheaper than Costco for the whole package. If the item was packaged
slightly differently (i.e., say an 8 pack instead of a six pack), it
would be a dollar or two more when adjusting for quanity.
They definitely are banking on people's inability to do simple math.

at Walmart because not everything qualifies for free shipping. I also do
this with Drugstore.com which is now operated by Walgreens.

For all of these places I also get Swagbucks because I am signed up through
them. Through my searches and shopping, last month I earned $25.00 of
credit from Amazon.com and also donated some money to hurricane Sandy. I
still had more Swagbucks left over which I just redeemed. You can only
redeem 5 of any one prize in a month. All in all these things are money
savers for me!

One of the age old questions is whether or not it pays in the long run to
be a driver of competition by rewarding the lowest priced vendor
through buying from them even if, occasionally, it costs a bit more
when everything is considered. I've always felt that it works out for
the best whenever I do that. Mindless selfishness seems to me to
inherently work against the mindlessly selfish.

Besides, I never make the 60 mile trip to Walmart for one item from one
vendor. Through planning and combining my purchases, I save even when
one or two items cost a little more than they would locally, and I
concurrently encourage market forces to work in my favor.

But then, of course, with no imagination available to duplicate such
achievement, nothing works well for some. A friend told me years
ago that "the poor always pay more." It seems to me that poverty in
vision costs more than all other factors combined. I admit that on
occasion that characteristic manages to warp around itself to bite me,
but I do the best I can to keep it from happening. Being human means
I don't always succeed.

OTOH to analyze every transaction as an isolated event guarantees
failure. Alas there's no cure for stupidity, and that's sure a stupid
way of looking at things.
