Why does the batter sometimes, not having hit the ball at all, start running
for first base...and...usually get put out?

If the batter has 2 strikes and he gets a third..the catcher must catch the
ball-if he does not he must throw the ball to first base and before the
hitter -and the hitter is out..if the hitter beats the throw he is safe at
first with a passed ball.

Only when first base is unoccupied, or occupied with two out.
