And how many of those payments were made because SSA didn't update its
records when notified of a death? In other words, how much is due to
bureaucratic ineptitude rather than fraud?

Good point. I found out today that the institutions where I had my brain
surgury and rehabilitation could not produce the documentation of when I was
there. Yet I've got a pile of papers from SSDI determination process with
their reports all throughout.
Good thing I held onto that stuff!

That makes perfect sense. He isn't dead, so his spouse (or former spouse in
certain situations) isn't a widow and isn't entitled to survivor's benefits.
