Cant find Medicare Part B Cancel form

Hi all.

My wife started SSD back in Feb, we sent in the cancle part b form in
March and they are STILL taking out for part B. I cant find the copy
that I made and was wondering what the form number was or if anyone
can provide a link to the SSA or Medicare website that contains this
form for download so that we can resign it and send it in by certified
letter. (even though we used their self addressed return envelope)

My advice is to keep your Part B. There are two things you can't have
enough of...insurance and....uh..never mind the other

We dont need B right now. She is on my insurance through my work.
Right now its just pissing away $96 a month for nothing. We can get
her back into Part B with no penalty if my insurance quits or I lose
my job.
