Medicare Lost Physical Exam

My Cousin Jim had a physical exam to qualify for medicare disability.
Now they say they have lost the file.
They can't do any counseling until they find the exam.
e thinks they don't want to award him disability,
His wife says he should go to the doctor who examined him and have
another exam.
