Piggybacking on Bill's reply, in my experience OTA HD is best by a
slim margin. Running a fairly close second, close enough that it
doesn't bother me or anyone else in my house, is Dish HD, DirecTV HD,
and Comcast HD. I can't speak for all cable systems since I don't have
any relevant experience with them.
About 98% of the time I can't tell any difference between the two
satellite providers, Comcast, and OTA. (Still comparing only HD.) The
only place I notice it, and it's not all the time, is during very dark
scenes or scenes that have a very limited palette. In those cases, I
sometimes see 'banding' from the second place finishers, while I very
rarely see such a thing on OTA. The local CBS affiliate is especially
clear here, probably because they're the only major network without
subchannels.
