Folks had been doing it for years, long before Heil n Ross/Kustom.
What folks forget to do, was pad the signal down. Before Heil/Kustom,
they would take any old driver and attach a tube, and shove said tube
down the cake hole. Without a pad, signal in ya mouth, down ya gut,
and forced into ya lungs (depending how ya chewed on it) did harm.
Folks who smoked and put the tube past the gums had lung problems,
if the signal was too loud, db wise. One guy, IMMSMW, in Canada
sucked the tube down his throat, and died from lung punctures, as
the JBL 2" driver attached to the 300A power amp was a bit too much.
Besides, it's a stupid effect, only Frampton made $$ at it, and it
gets old, real fast. Heil still makes one, I think, for sale.
Heil pioneered alot of PA concepts. Get the book:

Joe Walsh is a ham? I had no idea! It's so un-rockstar-like... but he
seems to be a goofy and independent guy, so I shouldn't be surprise.
In the Bon Jovi case it was actually Richie Sambora using it.
