Did you know NTSB statistics attribute 85% of aviation accidents to
pilot error?
And it isn't limited to low-time pilots. We've all seen the reports
where seasoned pros commit fatal mistakes.
We can easily ignore this, saying "It will never happen to me." Or
we can meet it head on and make it a priority to learn how to
recognize and deal with the causes of pilot error.

Is that about the same ratio for us FS pilots???
Of the 2 accidents I have been in the one where I was on the controls
and also the Pilot In Command it was a malfunction of the flight
controls that brought me down. Reminding me of the sayin' -in a
helicopter emergency the altitude below you is not your friend-.
In my second accident I was in the front seat of a Cobra and the rear
seat pilot was on the controls. That one was pilot error. He caught the
tail rotor in the trees. I'm at 50-50 in the real world. All my
accidents in FS have been aircraft failure. Of some sort...
