I'm guessing that it was played on 6-12-93, See also following comment
on web page -

first interview was made in '90 NYC with mark and howie,
second interview(the one about PlaygrPsyc) was made "two months ago" in
sproutin wings LA,...so that would be oktober '93

So was "Supplement" not the title of Co de Kloet's program? Or was this
interview broadcast on a different program?
Do you feel that it's necessary to cite both the station and the
broadcaster? If not, which one should I cite?

It was indeed. I remembered the explanation of the situation, but not
the advice about what to cite. Looking back, I see that Desaab
recommended citing both the broadcaster and the station, as you have.

Supplement was indeed another program title. Co de Kloet made many
different programs over the years with one thing in common, a lot of
attention for all things Zappa.
