Are these SW-21's intended for outdoor installation (as is the case with the
DirecTV elliptical dishes) or is the length of cable from the LNB to the
SW-21 not as critical? (Eg. they could be situated 15-20 feet away within
the home).

From what I've seen, they're the same DISH SW21s anyone else gets. They're
able to withstand normal weather conditions (read here: designed to work
outdoors) but in reality, switches work better the closer to the receivers
they are.
For example, an SW21 mounted 10 feet from a receiver will support far more
than 100 feet of RG6. The closer to the dish you get, the less it will
support.

Yes, they are rated for outdoor service, but it's best to
shelter them from the elements if possible. Be sure to put
drip loops into the wiring going to and running away from
them to avoid water damage.

Good solid copper center conductor Quad Shield RG-6 should
be used to support long runs.
It's to your advantage to put the switches as close as
possible to the receivers, but if you mount them indoors,
you should not use them as ground blocks, mount real ground
blocks where the service enters the structure (and ground
them!).
