Correct answers not showing up any more for each different student

Have a little math program working on and as little tommy logs on some
questions he will get right and some he will get wrong.

Then when he logs on tomorrow I want only the ones he got wrong to
show up so he can have a go at them not the ones he got right. But
then if little sally logs on all the questions appear so she can try,
then as she gets answers correct they go away not to appear again,
only the ones she got wrong so she can try those again.

Any ideas on the code to do that?

How does little tommy answer questions on the computer since little
tommy maybe too young to know how to actually type out a sensible
answer?

I can only assume that maybe multiple choices that could be
checked off would be the correct way. But this will involve
some graphics so that little tommy can better understand the
question..

Maybe little tommy isn't so little and some school work is needed to
know how to write and read formatted files?
