KERRY O'BRIEN: Scandinavia and Australia may be on opposite sides of the globe, but they're linked by a scientific riddle - an
unexplained surge in the rate of juvenile, or type one, diabetes.

Over the past decade, the number of Australians with diabetes type one has risen by 37 per cent, while the number of children under
five contracting it has doubled.

One controversial theory holds that specific proteins in baby formula based on cow's milk could be to blame, which the dairy
industry disputes.

But right now, doctors and researchers are recruiting mothers and babies to participate in a worldwide study to settle the issue.

As Mark Bannerman reports, some families believe the question must be answered, even if it might mean putting their own children at
risk.

Mark Bannerman reports.

MARK BANNERMAN: His name is Bradley Clapham and like any other eight-year-old boy he's limbering up with a few games of virtual
soccer before school.

But Bradley Clapham is no ordinary boy.

He has type one diabetes and unless his blood sugar is monitored all the time he's in mortal danger.

That means starting with the first insulin shot of the day.

His day must be planned meticulously.

DONNA CLAPHAM: Every day is a military operation with him.

It's also, I mean, you sort of got to know what they're doing from hour to hour, or what activities they're doing during the day and
just, you hope that you are giving them the right amount of insulin to cover those activities and what they're eating for the day.

MARK BANNERMAN: And do you worry?

DONNA CLAPHAM: We always worry.

It never leaves you.

MARK BANNERMAN: Little wonder his mother feels a weight of responsibility, for Bradley Clapham was diagnosed with type one diabetes
when he was just 12-weeks-old.

Indeed, on the day he fell seriously ill, he was so young, the doctors in emergency at the hospital first did not believe he could
be diabetic.

The result was terrifying.

DONNA CLAPHAM: He was blue when the paediatrician arrived.

And she arrived out of a sheer hunch to come in early that particular morning at about 6:00am in the morning and she basically saw
that he was in distress and had died in my arms.

MARK BANNERMAN: Bradley was revived, and for the next few hours, hung between life and death, sustaining mild brain damage along the
way.

DONNA CLAPHAM: He died again on presentation at the hospital and I always remember the endocrinologist and the intensive care nurse
coming out and saying, "We only give you 20 per cent chance he will live."

So, I mean, that was devastating for us.

MARK BANNERMAN: This young man may have survived his initial encounter with diabetes but the battle is never quite won.

Each morning at 10:30 at school, he has his blood sugar tested.

His situation, though, is not unique.

In the past five years, the number of children under five diagnosed with the condition has doubled, and as any doctor will tell you,
the earlier you get type one, the more dangerous it is.

PROFESSOR PAUL MITCHELL, OPHTHALMOLOGY, WESTMEAD HOSPITAL: There's the problems of a person rapidly growing, the hormonal factors
particularly in girls - it makes control of diabetes particularly difficult, and it's the control of the diabetes particularly in
the first five years or so that a person has the disease, that is critical in terms of whether they're going to go on to get
complications from it.

MARK BANNERMAN: Of course, the question is why are so many young people coming down with diabetes earlier in life?

Well, scientists think they know the answer to this and it involves the food we feed our children for the first six months of their
life.

In particular, the scientists are looking at baby formula based on cow's milk.

Could it be a trigger for type one diabetes?

The case against cow's milk protein begins in the 1960s.

As women were encouraged to feed their children on cow's milk formula, the incidence of type one diabetes began to rise.

DR NEVILLE HOWARD, PAEDIATRIC ENDOCRINOLOGIST: If you look at children getting diabetes in a study and ask about their infant
feeding, there is a relationship to early weaning.

MARK BANNERMAN: In theory, the process works like this.

When the macro proteins from cow's milk formula enter the baby's body, they head for the gut.

The baby's immature system allows the macro proteins to leak through the stomach lining, setting up a chain reaction where the
proteins are attacked by antibodies.

The problem is it's believed these same antibodies can carry on to destroy the insulin-producing islet cells in the pancreas,
bringing on diabetes.

If this is true, the implications are profound.

Understandably, the dairy industry is deeply concerned about such allegations.

DR ANITA WELLS, DAIRY AUSTRALIA: We believe it is just a hypothesis.

It's not proven and more recent, more robust, research has shown that there's absolutely no link between type one diabetes and cow's
milk infant formula.

MARK BANNERMAN: But scientists will not be put off.

Right now in 20 countries around the world, they're looking for mothers and babies to take part in a major trial to find out once
and for all if cow's milk protein can cause diabetes.

It's called the Trigger Study, and Dr Neville Howard is the man leading the research in Australia.

So does he think milk might be the villain?

DR NEVILLE HOWARD: Well, yes, and that's why I put my money into the study and my time and our resources, because we have
information from this part of the world.

Our epidemiology did show a significance of cow's milk feeding as a factor in the development of Australian kids with diabetes.

MARK BANNERMAN: To prove this assertion, you need to find a very select group of babies.

They must have a parent with type one diabetes, they must have a genetic marker for type one diabetes themselves, and significantly,
the mother must be prepared, if they cannot breastfeed the baby, to give their child a formula that may or may not contain cow's
milk proteins.

Ryan Carson is just four weeks old and he's on the Trigger team.

His mum, Rael Carson, is a diabetic, diagnosed at just 11 years of age.

RAEL CARSON: For some diabetics during their teenage years, your control is not very good because of the hormonal changes that are
happening within your body.

The hormonal changes affect the way the insulin is actually processed and that sort of thing, so I spent a lot of time in hospital
sort of over a period of three years.

MARK BANNERMAN: But Rael Carson's problems with diabetes did not end in her teens.

Today, she's come to see ophthalmologist Dr Paul Mitchell.

Due to her condition, extra blood vessels are growing in her eyes and they could haemorrhage at any time.

PROFESSOR PAUL MITCHELL: It really requires to be detected by an examination.

You can have proliferative retinopathy develop, not having been examined, and then have a haemorrhage and lose vision quite quickly.

MARK BANNERMAN: The good news - there is a treatment.

The bad news is it destroys her peripheral vision.

RAEL CARSON: Overall, it's to save your vision completely.

I'd rather have sort of diminished vision rather than no vision at all.

MARK BANNERMAN: Perhaps because of this, Rael Carson and her husband, Ray, had no hesitation in joining the Trigger study.

But their decision should not be underplayed.

Knowing that cow's milk may be a factor in diabetes type one, they are allowing their child to be part of a test that may in turn
expose him to that very trigger.

RAY CARSON: I didn't have a single misgiving.

Eat Breakfast, Cut Diabetes Risk

A recent study has revealed that people, who eat breakfast every day, are less likely to develop obesity, type 2 diabetes or even fat around the tummy. Presented at the annual meeting of American Diabetes Association, the study includes over 5,000 men and women, none of whom had type 2 diabetes when they entered the study.
