1. Updated to output the *_GLOBAL opcodes correctly. More specifically,
indexing into global table is supported for variables too.
2. Added SETUP_LOOP to the list of stop codes.
3. Made opcodes loading boolean values as LOAD_NAME instead of
LOAD_NAME_GLOBAL, since "True" or "False" will be in the local table only.
