We have a product named HEAT, this tracks all calls that come in to the Service Desk. We log everything in these tickets. The customer can call or e-mail the Service Desk and tell them their request. This is then assigned to the appropriate IT person.

"hi ganesh, we need to know how well we're doing in copay collections. you gave us something that looked at the insurance screen and cashier transactions for ER while ago that is working out well. we need something to run on a monthly basis for PT/OT, but keep in mind these are recurring accounts with a copay for each visit. when you have this done by? call me if you any questions..."

Hi Diane,

We use a "Request for Service" form that is posted on our Intranet. We then track the RFS through our helpdesk system so we have all the documentation for it. Form attached, in MS Word format.
