[MT-L] One time orders and Pyxis

Hello,

If an order is entered as a ‘once one’ order meaning a frequency of once and a schedule of one, are users able to pull the med out of pyxis even though the order discontinues after 1 minute?  Right now our practice in magic is to give a window of time (say 8 hours) to get and give the med prior to discontinuing the order, but this gets to be a real pain especially in order sets.  As I am building 6.0, I would love to be able to go to a once, one scenario. 

Thoughts?

Yes, if your Pyxis is set up to allow meds to be vended for a period of time (I usually see 1 -2 hours) after it id DC’d then ONCE and ONE works great. Using ONCE – ONE also allows for the order to auto DC from the eMAR immediately after it is documented. This is safer than giving a window of time since the system doesn’t count the doses and it would be possible to administer/document several “one-time” doses within that time period.

It's a Pyxis setting, how long after the time to allow for a vend.

I had our Pyxis integration engineer build a unique Order Lag Time for just one time orders.  They can build that into the .INI file on the Pyxis side.  This will keep the ONCE ONE orders from discontinuing after one minute.  If you are going to CPOE later, it is a must to use ONCE ONE orders.  Hope that helps.

In our experience, the Lag Time setting in Pyxis (on the Rx tab of the Device menu) does not work with ONE orders. So the issue is not one that you can solve yourself. As Tim explained, your Pyxis Integration Engineer will need to edit the settings on the Pyxis console to accommodate ONE orders. We have ours set to drop from Pyxis after 6 hours, or after the med is vended, whichever comes first. One caveat that we have discovered: When an ER patient is admitted in Meditech, any undispensed ONE orders are immediately dropped from Pyxis. Other than that, it has worked well for us, including Order Sets in Pharmacy and POM. One point of clarification: not all ONE orders DC after one minute. If you are dispensing IVs from Pyxis, Meditech takes into account the infusion time, and sets the DC at the end of the infusion.

 

We had struggled (I’m the one who had to hard-code DC times on every med for all one-time orders in QuickScripts) with this issue since the beginning, until Pyxis was able to solve it. Pyxis always said that the reason the Lag Time setting doesn’t work with Meditech is because Meditech sends a hard DC message for every order, but the Pyxis Lag Time is designed to work with the DC time attached to the original order.
