During our recent Lab JCAHO survey, it was brought to our attention that our
Lab reports do not have the Verified Date and Time for each test on the
report.

There is a prompt in the Lab Result Format Dictionary to set this to Y,
however we are running into issues where this sometimes prints on the next
line, causing the report not to look as clean and it also lengthens the
report.

Any suggesstions?

We are C/S. Are you saying they want the verified information on the
external copy of the lab report? We just show them an internal report
where all of that information is visible if they ask for that
information.

I believe the intent is for the date and time of reporting (preliminary or final) to be present on the report. If the date and time can be tracked for each analyte (ie on the internal report), that is sufficient for the audit trail.
We added last.activity.date and last.activity.time on the LAB Specimen Header for the INQUIRY report. This will show the date and time that a preliminary specimen was resulted and then change the date and time to reflect when the specimen was completed.

We just had our JCAHO visit and the verified date and time on the header was not an issue with them. We are also accredited by CAP. We can still produce each analyte's verified date and time from the internal inquiry report if asked. As you stated, some surveyors may over-interpret regulations.

That may be the intent, but a growing number of sites accredited by JCAHO instead of CAP are having surveyors tell them they want to see the verified date and time of EACH ANALYTE result. Our lab is CAP and it hasn't been an issue so far. As a former Lab Director in a JCAHO accredited lab, I can attest that some JCAHO surveyors tend to over-interpret some of the regulations., losing sight of the intent of them.

We ran into the same problem with the Joint Commission. Our MD's HATE the
summary reports. We were already reporting the verified date and time on
specimen reports. I would love to hear some "workarounds" or different
formats that would not be so "busy"

We are a CAP lab within a JCAHO accredited hospital and we had to deal with this at our last JCAHO inspection. CAP just left here Wednesday and they were fine with seeing it on the internal inquiry. For JCAHO we added the verified info to the end of the summary report that goes to the ELR (we are C/S 5.64) but we did not add it to our specimen reports.
