OK, so I scored a used Xbox 360 for our gaming events. I'd like to try
out a Halo 3 tournament and saw that some libraries have 17-under
contestants' parents fill out a permission slip. Does anyone here have
a copy of these permission slips that I can adapt for my gaming
events?

Never mind. I've just been told that I'm not allowed to have any M-
rated games for my programs. Lame.
