Web site service agreement Scope of Work

The SCWMA is seeking to establish a monthly web site
maintenance
service agreement Scope of Work with a web site design
company.

Does anyone have a sample Scope of Work agreement that
answers
questions like these:

1) How much time will the contractor spend maintaining the
web site per month?
2) How much space will the contractor provide to the web
site?
3) How much traffic will an agreement cover?
4) Is any new web development covered? If yes, how many
hours/month are covered?
