I've seen some relaly wacky ones. There are sites dedicated to funny
404's.
I have a mildly funny one one one site. I couldn't brink myself to be

Actually, that 404 page does return the right result code, but only
for URLs that don't exist :-). Technically, the page shown exists at /
404.shtml, so in a way, it can return 200. Notice that any other bad
URL on that site does return the proper result code: http://www.bluedaniel.com/badurl
Apache (and I believe IIS as well) handle 404 errors that way -- they
have a page which exists, which is returned instead of the missing
page. The result code is created by the server and not by the page. In
other words, if you don't know the name of the actual 404 page, you'll
always see a 404 result code.
Now whether or not that page should be indexed I guess is a different
question altogether. To be or not to be ... :-)
