Images are indexed based on the pages where they appear.
So if a page has any kind of content which qualifies for being
restricted with safe search on, that is what needs to be pointed out.
Use ICRA and/or PICS labels appropriate to each page's content for one
thing.
No idea how fast Googlebot reacts nor what signals exactly it takes to
judge content, especially for images.
is google actually using ICRA and/or PICS labels ?
some people reported that it was "safer" to place all contention
images in a separate folder, because google would black-list the
folder if one image was reported as "explicit" popping-up in safe
search. is that the right approach? or is it better to move all
contention content to a separate domain?

Yes, as webado mentioned, you can split these things into separate
folders, which will help us to somewhat understand the differences
(especially if pages and images both have their "adult" folders or are
in a shared "adult" folder). That said, I personally find it somewhat
problematic to have a site with lots of adult and non-adult content:
how easy is it for someone who stumbles into the non-adult part to
reach the adult part? Perhaps it would make more sense to split it
into subdomains or even separate domains? Anyway, that's hard to guess
at without knowing the site in question; maybe splitting things into
separate folders is already good enough :-).

I think from a practical point of view, even the non-adult images
(talking about the landing pages, which are shown in Image Search) are
just a click or two away from adult images. This makes things somewhat
difficult to manage: if someone is searching with safe-search turned
on, they generally want to land at sites that are non-adult and do not
wish to be surprised by adult content just around the corner, a click
away.
Perhaps it would make more sense to organize your site into two
distinct parts - one with adult content and one without it? I realize
that as a photographer you might prefer to keep all of your work
together in one place (at least that's probably what I would want),
but in this case I imagine there's no way around saying that the whole
site is for adults.
