I am trying to turn off automatic filtering by setting the filter
parameter to 0 in the link to the default search form. However, this
does not have any effect - the search results still show only the 2
most relevant items. Is there a trick to make this work?

Glenn,
You would also have to modify the xslt frontend to carry this
parameter on to the next page. AFAIK the filter parameter is not
carried forward to the next query.
Note: the parameter however gets carried over if you click "Next"

There are a several items that the frontend handles like the filter
parameter. I would suggest ensuring all of the features that you want
carries over are not listed.

Any instruction you could provide on what specifically in the xslt
needs to be modified would be greatly appreciated. It is not obvious
to me what needs to be updated.
