I found that exporting the file from Thunderbird to a .csv file and
editing it in excel was time consuming.  It took some time to clean up
the clutter.
Some experience in excel is required.

Most important: You MUST make header names for each column.
Save the excel file as a csv file without changes. (Say YES)

Then, the import worked for me.

When you open the CSV file that you saved from the mozilla or outlook
expess file, does it contain a header line with subjects that
correspond to the comma separated values in the address book?
For example, I exported a CSV address list from Mozilla Thunderbird to
a file on my computer, but it only saved the email addresses and names
without a header at the top showing what those files were. I edited the
saved CSV file and added the following categories to the top line;
First Name,Last Name,Display Name,,email,,work phone,home phone,,,cell
phone,street address,address 2,city, state,zip,,,,,,,,,,,,,,,,,,,,,
There's probably a way to tailor your export so that you don't have so
many commas, but each comma separates a field, so the header line has
to have as many fields the address fields that were exported. Once you
add the header line, you should be able to import it to Gmail. I ended
up importing it twice because I didn't know there were 2 tabs on the
contact list; one for frequently sent contacts (which was empty since I
hadn't sent any emails yet) and the one next to it that shows all
contacts. Click on "all contacts" to see if the import worked.

I was able to get the import to work after inserting the following (as
a single line) at the beginning of the CSV file exported from Mozilla's
Address Book -- use of Excel was not required):

first name,last name,display name,nickname,email address,alt email
address,
work phone,home phone,fax,pager,mobile,address1,address2,city,state,
zip,country,work address1,work address2,work city,work state,
work zip,,,,organization,,,,,,,,,,notes

By the way, there's a known bug in Mozilla 1.7.5 that prevents it from
exporting its Address Book as a CSV file. I worked around this by
installing the latest 1.8 beta test version which has the problem fixed
in it. Don't know if this is an issue with Thunderbird...

Hope this helps.

Martin: if the use of Excel was not required, where did you paste the
contact list to be exported?  I'm having too much trouble with using
Excel to make it worth while.

CSV files are just text files formatted in a certain simple way, so I
just used my favorite text file editor. Notepad (or many other
programs) would also be fine.

P.S. "CSV" means comma separated values.
