YAM, NASEEM AND FIROZ VELLANI.
Samiah our thirteen year old has a rare form of a plastic anemia which
leads
to progressive marrow failure. As per her hematologist she will likely
require a marrow transplantation over the next few months. Finding a
matched
donor is extremely difficult. The odds I believe is well in excess of one
in
twenty thousand. The likelihood of finding a compatible match is mostly
within ones own ethnic group.

There are 12 million donors in the international bone marrow registry but
south Asians comprise less than 1 % of this total. India has a small
registry which is part of the international registry. Pakistan has none. In

the U.S. there are about 7 million registered donors of which south asians
number approximately 130,000 only. There is an urgent need therefore to
expand the marrow registry in India to meet the increasing needs as bone
marrow transplantation is the definite cure for some otherwise fatal
conditions. Over the last few years tremendous progress has been achieved
in
the field of bone marrow transplantation. If the right match is found
potential cure is possible for patients with various forms of blood
disorders including leukemia, aplastic anemia and thalessimia. Speaking of
thalassemia, it is a relatively common in certain parts of the world
including the Indian subcontinent. Bone marrow transplantation offers hope
for cure in this devastating genetic condition.

Bone marrow transplantation is now more and more decribed as marrow
transplantation. This is because in over 85 % cases transplantation is
with
peripheral stem cells. This is how the process works. The donor after
recieving filgrastim a drug which stimulates the bone marrow to produce
increased numbers of stem cells is connected to an IV. The donors blood is
then filtered through an apheresis machine which is much like a dialysis.
Some of the stem cells is obtained and the blood is then returned back to
the donor through an IV in the donors other arm. The donor does not need
any
sedation, in fact could be sitting up while the procedure is ogoing. It
takes an average of 2-4 hours to complete this. Filgastrm is a very safe
medication. Sometimes it may cause a mild transient bone pain or cold
symptoms. Further info can be obtained at www. marrow. org.

The challenge we have is how to increase the donor pool ? I am confident if
our Jamaat is provided the requisite knowledge and made aware of the dire
staits that these patients and their families are confronted with they will

Insha Allah be galvanized to register as donors. With the help of our
Health
Boards national and local, volunteers, friends and family members as well
as
the local media both print and electronic we have conducted several drives
in the U.S. and have succeeded in registering over three thousand donors.
This is still a very small number. With the grace of MHI we have benefited
a
few patients including a young man in Europe ! The donor was identified in
Houston . Peripheral stem cells was obtained through the process described
above at a local hospital. The specimen was then shipped to Europe. The
respective national registries cooperate very well under the ageis of the
International Bone Marrow Registry.

We hope and pray that our Health Board in India with the help of local
experts will act quickly to develop a programme to create an awareness in
our Jamaat and the at large community, and succeed in enrollling donors for

this life saving procedure. Too many lives has been lost for not being able

to find a donor. My wife is a registered nurse and I am a pediatrician. As
we have come to be recognized as parents actively involved in this process
we have recieved calls from some jamaati members including from India for
their loved ones both adults and children in need of a bone marrow
transplant.

We have at our disposal funds if needed to implement a programme to help
our
daughter or any jamaati member. A web site has been established for Samiah
at www.Samiahssavior.com. We look forward to hearing back soon. YAM.

Dr. Raza Ali Razak
