Google Voice unable to dial in or out, error: "Cause: DESTINATION_OUT_OF_ORDER"

As the subject says, I'm using GV for both in & outbound calling.
Originally I was having an issue with the fact that mod_dingaling was
not registered which I fixed by issuing load mod_dingaling in the FS
CLI. However, whenever I try to dial out from the internal account
that is linked to GV, I get this:

I asked on the IRC and munson couldn't recall the fix. I just pulled
and compiled the latest version off of Git not too long ago (few
hours), so I don't believe I need an update. I'm configuring this
using the latest (as of this writing) version of Blue Box and followed
the guide here: http://wiki.2600hz.com/display/bluebox/Integrating+with+Google+Voice+%28with+blue.box%29
which up until this point has been pretty fluid.

I'm new to BB and this whole thing so I'm not sure what should really
be mentioned in here. GV is set up with call screening on, call
forwarding off and chat on. All the authentication set up in BB is
correct and I can log into the FS account just fine.

You need to repost this in debug log level 7. This isn't enough
information.
